NHS England Examples as HL7 v2

These are HL7 v2.5.1 ORU^R01 versions of the examples in the NHS England Pathology FHIR Implementation Guide - Examples Index (version 0.5.1). They show what a LIMS sends (Order Results Management [LAB-3]) for the same reports that laboratory reporting systems send to the NHS England Pathology Laboratory Reporting API as FHIR Documents.

They were generated from the FHIR Documents in the fhir.r4.nhsengland.pathology package by scripts/nhse_document_to_oru.py, using the FHIR to HL7 v2 rules on Mapping HL7 v2 and FHIR. The notebooks convert them back: ORU^R01 to a FHIR Message, then the FHIR Message to a FHIR Document for the NHS England API.

The Three Forms of Each Example

Each example below is given three ways:

  1. the HL7 v2.5.1 ORU^R01, as a LIMS sends it (LAB-3)
  2. the FHIR Message converted from it - the FHIR equivalent of the HL7 v2 message, as in Laboratory Result (R01). Like HL7 v2, it refers to practitioners and organisations by their codes (identifier references), not as resources
  3. the FHIR Document built from the FHIR Message - what the order comms system sends to the NHS England Pathology Laboratory Reporting API

All the FHIR examples are also listed under Artifacts (groups NHS England Examples - FHIR Messages and - FHIR Documents).

Differences from the NHS England FHIR Documents

The results are the same as NHS England's - every Observation code, value, unit, reference range and flag. The FHIR Documents here are not copies of NHS England's, though: they are what a system produces when it builds the document from an ORU^R01 following this guide. The differences, and why:

Human readable narrative - the most important addition. A FHIR Document is a record a person reads: one of the core principles of FHIR Documents is that the document's narrative (the Composition and section text) is the attested content, and that a document must be displayable to a person without the receiver understanding every coded element. NHS England's examples leave every section narrative empty, so a receiver has nothing human readable to show. The documents here generate it from the coded resources: a header in Composition.text (NHS Number, report number, status, requester, laboratory, specimen, issued time) and a results table for each test group, plus the report comments - see Human Readable and Computer Processable. That makes the documents humanly readable, which they must be for a GP system to display, print or forward them safely.

Other additions - they all come from content that is in the ORU^R01, so leaving them out would lose information:

  • a Patient resource with the NHS Number, hospital number (MRN) and laboratory patient number (PID-3) and demographics (PID-5, PID-7, PID-8); NHS England's examples have only the NHS Number
  • order details on each ServiceRequest: the test requested (code, OBR-4), the Filler Order Number (ORC-3), identifier types (PLAC, FILL, ACSN) and the ODS code of the assigning organisation (assigner)
  • DiagnosticReport.effectiveDateTime - the specimen collection time (OBR-7)
  • a second code where the meaning is the same: LOINC 11502-2 Laboratory report alongside SNOMED CT 721981007, and the HL7 v2 table 0074 discipline (OBR-24) alongside the SNOMED CT specialty
  • a report comment section when the report has a comment (NTE after the OBR: the GTT and urine examples)
  • references that carry the identifier and name as well as the reference, so a receiver can match the practitioner and organisation without resolving them

Consistency - the conversion takes each value from one place in the ORU^R01, which makes some values consistent that are not in NHS England's examples (not intended as corrections, but worth knowing):

  • Composition.date is when the report was issued (OBR-22). NHS England uses the same date in all nine examples, in one case before the report was issued
  • every result's issued is the report's (OBR-22); two NHS England examples carry dates from other examples
  • ServiceRequest.status is completed for a final report (ORC-5 CM), not active
  • one section per test group: in the Lipids/HbA1c example NHS England has a single section and the HbA1c result is in none

Not carried through HL7 v2 (so not in these documents): the valueset-reference extensions on result codes (which NHS England list a code is from - they can be re-derived from the code), ServiceRequest.category, PractitionerRole.code and specialty, the FBC reason for request (could be sent in OBR-31), the GTT specimens' fasting status (no SPM field) and the Provenance agent type. The HbA1c interpretive reference range text becomes a result comment, because OBX-7 cannot hold several lines.

Profile - the documents declare England-Bundle-Lab-Pathology|0.1.0, as the API specification says (NHS England's examples declare Bundle-eu-lab|2.0.0). The published version of that profile has a slicing defect (see the Pathology Laboratory Reporting API page), so the validator reports slicing errors for these examples that come from the profile, not from the documents.

Notes

  • Patient - the NHS England examples identify the patient by NHS Number only, but PID-5 (name), PID-7 (date of birth) and PID-8 (sex) are required in HL7 v2. These messages use NHS England EDI test patients instead - the patients used by older versions of NHS England PaLM and by the NW GMSA IG. They exist in the NHS England INT environment (PDS), so developers can trace them:

NHS England example NHS Number EDI test patient Sex Date of birth Postcode
    972 771 0638 (HbA1c, CRP, Lipids/HbA1c, LFT/U&E) 999 999 9468 EDITESTPATIENT ONE (John) M 1925-01-27 B6 5RQ
    972 771 0859 (HBsAg, HPV, FBC, GTT, urine MCS) 999 999 9549 EDITESTPATIENT NINE F 2002-02-02 EX1 2SS

PID-3 also carries a hospital number (MRN, type MR, assigned by York and Scarborough Teaching Hospitals RCB, the local acute trust for the requesting practice) and the laboratory's own patient number (type PI, assigned by Y8J7D); these are recorded in the NW GMSA MRN-Mapping:

NHS Number MRN (RCB) Laboratory patient number (Y8J7D, PI)
    999 999 9468 RCB4719263 Y8J7D0381527
    999 999 9549 RCB6052841 Y8J7D0592374

PID-32 = 01 (NHS Number present and verified). The requesting practice and laboratory are those of the NHS England examples

  • Identifiers - order, report and specimen identifiers are sent as EI value^namespace, the namespace being the ODS code of the organisation that assigned the number. The FHIR Identifier.system (a URI) is not sent: the LIMS does not know it at this stage of the pipeline. The receiver - usually the integration engine - assigns it from an agreed table of (field, namespace) to system, as notebook 01 does
  • Test groups - each ServiceRequest becomes an ORC/OBR. A test group (an NHS England organizer Observation, for example the FBC) is the OBR and its members are the OBX. A single test is an OBR with one OBX
  • Report - all the OBRs share the report number (OBR-3 = DiagnosticReport.identifier), so the receiver can rebuild one DiagnosticReport
  • Text results - multi-line valueString (the unstructured examples) is an OBX of type FT with \.br\ line breaks
  • Reference ranges as text - interpretive text (the HbA1c example) does not fit OBX-7 (ST, no line breaks), so it is sent as a result comment (NTE after the OBX); converted back it becomes Observation.note
  • Character set - MSH-18 = UNICODE UTF-8 (the examples contain non-ASCII characters)
  • Units - OBX-6 is UCUM code^unit^UCUM when the example gives a UCUM code, otherwise the unit text only (^unit)
  • Not carried - specimen fasting status (GTT) has no SPM field in HL7 v2.5.1; OBR-24 is chosen for each specialism (the NHS England examples use the SNOMED CT specialty 394915009 General pathology throughout)
